What are an intake and case history?

Within my practice for coaching, sound therapy, phonophoresis and cupping, I always work with a careful intake and, where needed, a case history.

The intake and case history are intended to gain a clear picture in advance of the client, the request for support and relevant factors such as health, resilience and any contra-indications.

This is done so that every session can be carried out safely, attentively and professionally within the framework of complementary care.

The intake is not a medical diagnosis and does not replace an assessment by a doctor or other conventional healthcare provider.

Why are an intake and case history important?

A careful intake is essential for:

  • safeguarding the safety of the client;
  • identifying possible contra-indications;
  • being able to apply treatments and coaching responsibly;
  • tailoring the intensity, duration and form of the session;
  • working within the professional and ethical frameworks of CAT and BAT;
  • limiting risks within the professional practice (BAT safeguard).

How do the intake and case history work?

1. Completing it in advance

The client receives an intake and/or case history form in advance. This is completed fully, carefully and truthfully.

2. Assessment beforehand

On the basis of the information provided, an assessment is made of whether participation is responsible and appropriate. If needed, a session can be adjusted, postponed or advised against.

3. Attunement during the session

During the session, attunement remains central. Intensity, technique and duration can always be adjusted based on the client's signals.

Responsibility of the client

Participation in sessions within this practice takes place on the basis of voluntariness and personal responsibility.

This means that the client:

  • is responsible for completing the intake and case history fully, correctly and truthfully;
  • does not withhold any relevant information about health, resilience or contra-indications that may affect the safety of the session;
  • remains responsible for indicating their boundaries and bodily signals during a session;
  • understands that effects may differ from person to person and that no guarantees are given;
  • may decide to stop a session at any time.

Safety, BAT and professional liability

Within the practice, work is carried out according to professional standards and guidelines within complementary care (CAT).

In addition, I am insured through a professional liability insurance (BAT). This insurance forms an important additional safeguard within the professional practice.

The combination of:

  • a careful intake and case history
  • a professional approach within CAT guidelines
  • and professional liability insurance (BAT)

contributes to a safe and responsible practice.

Refusing or adjusting sessions

On the basis of the intake, I reserve the right to:

  • refuse a session;
  • adjust a session;
  • or refer to a general practitioner or other healthcare provider

when this is in the interest of safety or when a session cannot be carried out responsibly.

Confidentiality and the GDPR

All information shared during the intake, case history and sessions is treated strictly confidentially.

Data is processed solely for:

  • the safe delivery of treatments and coaching;
  • the attunement of sessions;
  • administrative and legal obligations.

Processing takes place in accordance with the GDPR (General Data Protection Regulation). Data is not shared with third parties without consent, unless required by law.

Application within the practice

The intake and case history are applied within all forms of guidance, including:

  • coaching and personal guidance
  • sound therapy and phonophoresis
  • cupping therapy
  • foot balance and relaxation rituals
  • vitality programmes for businesses and organisations
